Provision of personal health care services to assist clients in maintaining healthy lifestyles through preventive health screenings and exams, early detection and treatment of chronic disease, and referral to appropriate human service providers.

FEES & INSURANCE:

The CCPHD clinic accepts any insurance a client presents. However, if the insurance does not cover the service, the client will be responsible for charges depending on the sliding fee scale, except for immunizations which will be billed at 100%.

Fees charged for most services are based on a sliding scale according to income guidelines and/or availability of insurance coverage (private, Medicare and Medicaid). All clients will need to bring in proof of income so that we may apply our sliding scale and determine your total charges for the visit.

For clients with insurance, you will be expected to pay your co-pay at the time of your visit. If you are unable to pay your co-pay, we will help you reschedule your appointment. If you have any questions, please feel free to call 919-742-5641 to speak with a staff member.
